A 30-day, 6-month, and 1-year post-placement puppy check-in form. Surfaces placement issues early, captures testimonials with consent, and keeps your program connected to past buyers.
About this document
Most breeders mean to follow up with placements but never get around to it. The result is no early warning when a placement is struggling, no record of how puppies turned out long-term, and no testimonials to show future buyers what your program produces.
The Post-Placement Check-In is the form you send at 30 days, 6 months, and 1 year after pickup. Same form each time, sent by email, takes the buyer 5 minutes. It does three things at once. Confirms the placement is going well. Surfaces concerns early enough to help. Captures the photos and short testimonials that become program credibility for the next round of buyers.
What the document does:
It walks the buyer through eight sections covering how things are going overall, the puppy's health and vet care, behavior and training progress, daily life, and any questions for you. The final sections invite testimonials and photos with explicit consent options, including how the buyer wants their name attributed (first name only, full name, or anonymous).
The form is designed to feel warm rather than bureaucratic. Buyers are more likely to respond to a 5-minute form than a 20-minute one. The questions are framed as a check-in from someone who cares, not an audit.
What is included:
Format and how it works:
Word version for digital editing and a PDF version for printing. Most breeders email the Word version as an attachment with a short personal note and let the buyer type their answers in. Use the same form at 30 days, 6 months, and 1 year so you can see how a placement is evolving across the first year. Save each completed check-in by buyer name so the history is right there next time you reach out.
Why this version is different:
Most post-placement forms are afterthoughts. This one is built to do three jobs at once. Confirm placement health, catch problems early, and gather the marketing material your program needs without ever asking for it directly. The consent-based testimonial capture is what most breeders are missing. They have happy buyers and no documented testimonials, because they never asked at the right moment with the right framing.
Who this is for:
For breeders who want a real follow-up system instead of vague intentions to "check in someday." Especially valuable for breeders building a program reputation who need a steady source of authentic testimonials and photos. Pairs with the rest of the Buyer Pipeline documents and with any breeder website that shows past placements.

Updates
You bought version 1. When a state adds a registry requirement, a new federal rule lands, or a working breeder catches something that belongs in the document, the new file shows up in your inbox. Common questions
The contracts were drafted with an attorney and pressure-tested across multiple working programs, but they are templates — not a substitute for a lawyer for your specific state's laws. Each file ships with a clear note about that. Most breeders use them as a starting point and have a local attorney review for state-specific edits.
DOCX + PDF. The DOCX opens in Word, Google Docs, Pages, or LibreOffice. The PDF is fill-on-screen or print-and-fill. Everything is editable.
When I ship a new version of this document, the new file lands in your inbox. Laws shift. A state adds a registry requirement. A working breeder tells me something that belongs in the document. The next version goes out to everyone who bought the previous one. No upsell, no annual subscription.
14-day no-questions-asked refund. If the documentdoesn't fit your operation, you get your money back and I'd genuinely like to hear why so the next version is better.
Yes, by a wide margin if you need more than two or three documents. The Working Breeder Operations Kit bundles this and every other operations document for $22.
