A puppy makes more vet visits in the first year than most adult dogs make in five. Wellness exams, vaccination boosters, deworming, suspected limps, ear infections, anything-and-everything-eating incidents. Most owners walk out with a verbal summary and a credit card receipt, then forget the details within a week. What this document does:

The tracker is per-dog. Use one tracker per dog and start a fresh page once the first one fills up. Most adult dogs go through 1 to 3 pages per year. Puppies in their first year can fill the tracker faster.
